Research Themes
Development of a computed simulation program to train clinical judgement for fall prevention
The aim of this study is to develop computed simulation program to train clinical judgement for fall prevention.
A collaborative approach for the prevention of falls in stroke patients
Preventing falls in stroke patients during rehabilitation cannot be successful without effective collaboration. The aim of this study was to qualitatively evaluate a collaborative approach for fall prevention. The collaborative approach involved sharing values and attitudes (i.e., consciousness), education, and approaches, followed by integration and clarification of one’s own roles. We tested two teams comprising nurses (NS), physiotherapists (PT) and occupational therapists (OT) through simulation with stroke patients during rehabilitation. Descriptive data were collected via interview, and analyses used employing a framework approach. Answers to interview questions revealed that sharing consciousness, education, and approaches had the power to change, involving in recognition of “Sharing consciousness having the power to change towards a positive direction”, “Being able to understand other professions’ perspectives and ideas”, “Taking advantage of documentation provided by nurses”, and “Nurses documentation being a shared tool”. “Compensate phenomena among roles” was shown through integration and clarification of one’s own roles. These data suggested that a collaborative approach was effective in a qualitative analysis, quantitative analyses are planned for a future study.
Development of a fall prevention program adapted to the individual risks of elderly persons in facilities
The aim of this study was to develop a fall-prevention program adapted to the individual risks of elderly patients in a long-term care facility by increasing the caregiving skills and motivation of staff members through mutual action research. The program, developed through the synthesis of seven evidence-based practice studies, consisted of staff education, identification of problems related to fall prevention in the facility by the use of focus group interview, assessment of individual risks, implementation of care adapted to the risks, consultation about fall-related problems, and modification of care in case of falls. The fall-prevention program was shown to be effective, especially in reducing injuries. The fall rate per 1,000 residential days was reduced from 7.6 to 5.0 in the intervention ward, while it went from 4.8 to 4.3 in the control ward. The percentage of injured persons in the intervention ward was significantly reduced from 22.6% during the 6 months before the intervention period to 9.7% during the 6-month intervention period, and the number of injuries was reduced from 13 to 3. No one was harmed by the program. The process went from expectation and anxiety regarding an unpredictable process to feeling of achievement through role conflicts. Empowerment was considered a driving force. The change agents for staff members were the response of patients and family members to satisfactory care, and the recognition of the need for cooperation with other medical staff members. On the other hand, the change agent for the fall prevention team members was recognition of staff members who could identify individual risks and had potential care skills. In conclusion, the fall-prevention program was shown to be acceptable for use among elderly individuals in a long-term care facility by increasing the caregiving skills and motivation of staff members.
Fall prevention for the elderly in a long-term care institution: Development of an exercise program based on EBN
Our aim was to develop an exercise program for fall prevention, based on evidence-based nursing (EBN), to not only increase muscle strength in the lower extremities, balance, and mobility, but also to decrease fear of falling and to increase self-efficacy for fall prevention in the elderly in a long-term care institution. We developed an exercise program which consisted of a warm-up, static stretching exercise, muscle strengthening in the lower extremities, balance training (toe training and proprioceptive neuromuscular facilitation), and a cool-down, based on EBN. The study design was a clinical trial. We developed a program through two trials. In the first trial, the 30 elderly participants who agreed to our study were divided into an intervention group of 16 people and a control group of 14 people. The length of the program was three months, and we measured muscle strength, balance, mobility, fear of falling, self-efficacy for fall, and the number of falls to evaluate the effectiveness of the program. As a result, the intervention group showed increased balance and mobility and a decreased number of falls, although muscle strength and self-efficacy for fall did not increase and arm strength decreased significantly. No training-related medical problems occurred. We modified the program to increase muscle strength in the lower extremities, to increase or maintain arm strength and self-efficacy for fall. In the second trial, 40 elderly participants were divided into an intervention group of 21 people and a control group of 19 people. The results showed a maintaining of arm strength and an increase of self-efficacy for fall but no increase of muscle strength in the lower extremities. In conclusion, an exercise program for fall prevention in the elderly in a long-term institution may be useful.
Life management platform using social big-data for elderly home assistance
The purpose of this study is to develop the life management platform. This platform uses the remote sensing technologies and the analyzing technologies. The remote sensing technologies are a method of collecting behaviors of elderly and home environments by monitoring public utilities and using the variety of sensors. The analyzing technologies are a method of predicting assist information by collecting data.
